Community Wellness Psychiatric Rehabilitation Program is a trusted behavioral health home and psychiatric rehabilitation provider, proudly serving Baltimore, Maryland, and nearby communities. Our mission is to empower individuals facing mental health challenges by offering tailored care and comprehensive support, helping them achieve fulfilling and balanced lives.

Community Wellness began with a vision of care growing to support healing and resilience

Our leadership recognized that chronic homelessness often coexists with severe mental illness, which contributed to the disparities faced by many of our clients.

In 2013, we launched a mental health practice to support individuals in affiliate assisted living facilities. By 2016, it evolved into an outpatient clinic and psychiatric rehabilitation program, relocating to Highlandtown. Primary care services were later moved to a larger facility in Downtown Baltimore. In 2017, our mental health programs earned Joint Commission accreditation, followed by CARF accreditation in 2018. That same year, we expanded to offer health home services and opened a community outreach office in Anne Arundel County.

In 2013, a mental health group practice and freestanding general clinic was formed to service persons housed in affiliate assisted living facilities. Three years later, the practice devloped into an outpatient mental health clinic and psychiatric rehabilitation program. It relocated to Highlandtown at that time. The medical primary care services recently moved to a larger more suitable facility in Downtown Baltimore. The mental health programs were awarded accreditation with the Joint Commission 2017 and CARF in 2018. Later that year, the psychiatric rehabilitation program received approval to provide health home services and a community outreach office was opened to make services accessible to residents of Anne Arundel County.

Psychiatric Rehabilitation Services

Our psychiatric rehabilitation services focus on enhancing the skills and abilities necessary for individuals to live independently and thrive in their communities. Through a person-centered approach, we offer a range of rehabilitative interventions, including skills training, psychoeducation, vocational support, and socialization activities.

Individualized Treatment Planning

We understand that each individual has unique needs and goals. Our team collaborates with clients to develop individualized treatment plans tailored to their specific strengths, challenges, and aspirations. These plans serve as roadmaps for achieving personal growth, recovery, and wellness.

Medication Administration & Monitoring

Our skilled psychiatric professionals provide comprehensive medication management services to ensure safe and effective treatment for mental health conditions. We work closely with clients, their families, and other healthcare providers to optimize medication regimens and monitor progress.

Behavioral Health Home Services

As a behavioral health home, we offer integrated care coordination and support services to individuals with complex behavioral health needs. Our multidisciplinary team assists clients in accessing medical, behavioral health, housing, and social services, promoting holistic wellness and recovery.

Community Integration and Support

We facilitate community integration by connecting clients with resources, activities, and opportunities for meaningful participation in their communities. Through partnerships with local organizations and agencies, we help clients build social connections, engage in recreational activities, and pursue educational and vocational goals.

On-Call and Emergency Response

On-call and crisis intervention services are available 24 hours per day, 7 days per week, by telephone, to an individual who has been admitted to the PRP. Relevant staff shall assist the individual to access, as appropriate, the OMHC, mobile crisis, residential crisis services, hospitals, and other service providers that are designated to provide crisis and emergency care and treatment.

Linkage with an Individual in a Psychiatric Inpatient Facility or Detention Center

(1) If an individual who is enrolled in a program is admitted to a psychiatric inpatient facility or detention center, the rehabilitation coordinator shall make reasonable efforts to maintain ongoing communication with the individual and the individual’s inpatient treatment team.
(2) To the degree permitted by the setting, PRP staff serving the individual shall participate in aftercare or release planning from that setting.
