We specialize in psychiatric rehabilitation and care coordination, and we partner with a wide network of community agencies, healthcare providers, housing programs, and advocacy groups to ensure you get the support you need—even when it’s outside our scope.

 1) Housing & Supportive Living
Examples we refer to:

  • Emergency shelters and crisis housing
  • Transitional housing and rapid rehousing
  • Permanent supportive housing for individuals with disabilities/behavioral health needs
  • Rental assistance, eviction prevention, and tenant rights resources

Why it matters: Stable housing is often the foundation for recovery, medication adherence, employment, and school continuity.

2) Primary Care & Medical Services
Examples we refer to:

  • Family medicine and internal medicine clinics
  • Federally Qualified Health Centers (FQHCs) that serve regardless of insurance status
  • Specialty care (e.g., women’s health, pediatrics, cardiology)
  • Dental and vision care

Why it matters: Physical and mental health are inseparable. Managing chronic conditions supports psychiatric rehab goals.

3) Outpatient Mental Health (OMHC)
Examples we refer to:

  • Individual, family, and group therapy
  • Psychiatric evaluation and medication management
  • Trauma-focused, grief, child/adolescent, and other specialty services

Why it matters: Rehab builds skills/independence; clinical therapy provides diagnosis-specific treatment and symptom management.

4) Substance Use Disorder (SUD) Treatment
Examples we refer to:

  • Detox/withdrawal management and stabilization
  • Residential rehabilitation (short-/long-term)
  • PHP/IOP (Partial Hospitalization / Intensive Outpatient)
  • Medication-Assisted Treatment (MAT: methadone, buprenorphine, naltrexone)
  • Peer recovery coaching and recovery housing

Why it matters: Co-occurring mental health and SUD needs benefit from integrated, coordinated care.

5) Legal & Advocacy Services
Examples we refer to:

  • Eviction defense, housing court navigation, and tenant rights
  • Disability rights, guardianship, and public benefits appeals (SSI/SSDI, Medicaid, SNAP)
  • Criminal defense, re-entry support, and expungement clinics
  • Immigration legal referrals and community advocacy

Why it matters: Legal barriers can derail progress. Addressing them protects housing, work, benefits, and safety.

6) Linkage with Psychiatric Inpatient Facilities & Detention Centers
Examples we refer to or coordinate with:

  • Hospital and detention discharge planning and warm hand-offs
  • Continuity of care to outpatient treatment and rehabilitation
  • Re-entry supports for individuals returning from incarceration
  • Crisis stabilization and post-acute follow-up

Why it matters: Transitions are high-risk moments. Seamless linkage prevents gaps in care.

7) Additional Community Supports

  • Employment readiness, job training, supported employment, and vocational rehab
  • Education (GED, adult literacy, college readiness)
  • Transportation (paratransit, mobility vouchers, Medicaid transport)
  • Food security (food pantries, meal delivery, community fridges)

How Our Referral Process Works

Whole-Person Assessment

We assess medical, housing, legal, and social needs alongside behavioral health goals.

Tailored Matching

We identify trusted organizations that fit your eligibility, location, cultural/linguistic needs, and preferences.

Warm Hand-Offs

When possible, we contact the partner agency with you (by phone/email) and help schedule the first appointment.

Documentation & Coordination

We record the referral in your care plan and coordinate with your providers (with your consent).

Follow-Up

We check in after the referral to confirm access, troubleshoot barriers, and adjust the plan if needed.

Insurance

Many partners accept Medicaid/Medicare and commercial plans; some offer sliding scale or financial assistance.

Waitlists

Availability varies. If a program has a waitlist, we’ll suggest alternatives and interim supports.

Accessibility & Language Access

  • Interpretation and translated materials are available upon request.

Privacy & Consent

  • We only share information with partner agencies with your consent (e.g., signed Release of Information).
  • We follow HIPAA and applicable privacy laws. Your information is kept confidential.

 

Emergencies & Crisis

  • If you are in immediate danger or experiencing a medical/psychiatric emergency, call 911 or go to the nearest emergency room.
  • For urgent behavioral health crises, contact your local crisis hotline or mobile crisis team.
